Lockout/ Tagout program applicability

1. No LOTO procedure: Supervisor confirms how to correctly execute LOTO procedure and needs to make a new procedure after the task is finished
2. The LOTO program is less than one year: it is implemented according to LOTO standards
3. More than one year of LOTO procedure: Supervisor confirms the accuracy and continued validity of the guidance procedure

Also, employees should hang their safety signs on locks. Information such as the name of the lockup personnel must be displayed on the sign, and the date of more than one shift must be displayed.
Work carried over to other shifts requires a fully updated Lockout/tagout procedure.
Develop a Lockout/ Tagout procedure (OPL/SOP) for the equipment in 9 steps, which should be placed on the employee workstation or posted near the machine for easy inspection by the employee.
Confirmation of major

The factory shall establish a list of majors:

The major is responsible for filling in the LOTO license, identifying the energy source, identifying the energy source release method, checking whether locking is effective, checking whether the energy source is released completely, and putting personal locks on the energy point or the energy point lock box;

Contractor’s personnel are prohibited from being majors in any work performed by the Contractor alone/in which the Contractor participates; If necessary (such as box cutting line), additional approval by department manager and ES manager is required.
If majored in human maintenance, the LOTO permit must be confirmed by the machine operator.
