The term “Lockout tagout (LOTO)” refers to energy isolation and tagging and locking, which means before equipment maintenance, repair, or cleaning operations, by closing and isolating various energy sources (electricity, machinery, hydraulic, pneumatic, thermal, etc.) of the equipment, using special locks to lock the isolation devices, hanging warning signs, and clearly informing that the equipment is in a “prohibited start” state to prevent others from mistakenly operating and causing safety accidents. It is not a formalistic procedure, nor an optional step, but the simplest way to separate danger from life, using norms and rigor to hold the bottom line of safe production.
Looking back at countless safety tragedies in the industry, most of them stem from the neglect and slackness of this “death and life barrier”. In a certain workshop, the maintenance personnel sought convenience and did not isolate the running equipment, did not perform LOTO operations, simply turned off the switch and began the operation. Unexpectedly, someone mistakenly touched the start button, and the high-speed-running machinery instantly caused serious injuries to personnel; In a certain enterprise’s maintenance team, when multiple people were working collaboratively, they did not strictly follow the principle of “one person one lock, who locks it who unlocks it”, someoneremoved the lock,causing the equipment to accidentally start,resulting in an irreparable safety accident. These bloody cases warn us that every step of LOTO operation is about life and death， and cannot tolerate any or half-heartedness. A lock,a warning sign,seemingly insignificant,can stop danger at a critical moment and protect a life and the integrity of a family.
To build this “death and life barrier”,it is necessary to clarify operation norms and adhere to the responsibility bottom line. LOTO operation is never a single step,but a complete safety system with interlocking links and indispensable parts. First,comprehensively identify potential energy sources,understand the types and transmission paths of equipment’s energy,and not miss any energy source that may cause danger； second， precisely implement isolation measures,close energy switches,cut off energy transmission,and ensure that the equipment is completely in an “energy-free” state； then,standardize locking and tagging， lock the isolation devices with special locks， and hang clear and eye-catching warning signs such as “prohibited start” and “under maintenance”,so that everyone passing by or contacting the equipment can clearly know the risks； before the operation,conduct safety confirmation,repeatedly check whether the isolation is in place and the lock is firm,ensure there are no safety hazards before starting the operation； after the operation,the person who locked it must unlock it， confirm that the equipment is normal and all personnel have evacuated before restoring the equipment operation. This series of procedures,each step cannot be simplified,every detail cannot be ignored,this is the most basic respect for life,and also the most solemn commitment to safety.
To hold this “death and life barrier”, it is also necessary for all staff to establish a firm safety concept. The implementation of LOTO operation is not the responsibility of a single person,but the common mission of every operator and every safety manager. Frontline operators should abandon “experienceism” and ” psychology”,strictly follow each step of LOTO operation,without any “going through the motions” mentality,without any “won’t cause an accident”  . Safety managers should enhance regular training and supervision, popularize LOTO knowledge, standardize operation procedures, promptly identify and rectify potential safety hazards in operations, and make the LOTO norms deeply rooted in people’s minds. Enterprises should improve safety management systems and establish LOTO operation assessment mechanisms, and fully implement safety responsibilities at all levels, making every individual a guardian of this “life and death barrier”.
