An employee of the factory next door went inside the equipment last night to work. The machine suddenly started and the employee was trapped inside. He was sent to the hospital and could not be rescued.

Why does the machine suddenly start up?

All machines need energy to run. If the energy source is not controlled before operation, other employees’ misoperation will lead to accidental release of energy. Common dangerous energy sources in factories include electricity, mechanical energy, hydraulic pressure, gas, heat, chemicals and so on
To prevent accidents caused by the accidental release of these energy sources, we can divide our daily operations into two types
The first is to deal with simple routine repetitive operations such as bottles and the operation can follow the process of safe entry into the machine within the line of sight. The second is to follow the Lockout tagout process for maintenance and other operations with the risk of machine accidental start-up and uncontrolled energy accidental release.
First, let’s take a look at how to perform the safe entry process inside the machine:
1. Stop the device by using the switch on the control panel
2. Check that the device stops running
3. Use safety devices to isolate the device
4. Confirm isolation (such as device restart)
5. Troubleshoot faults such as card boxes and bottles
6. Restart the server
That’s how you get inside the machine. However, for maintenance and other operations, if risks cannot be controlled by this process, Lockout tagout is needed for management
