Considering the necessity of locking the signs with locks

Today, I came across a tragic accident case: On November 26, 2025, an automotive parts company in Shangrao City, Jiangxi Province, experienced a mechanical injury accident that resulted in one death. At around 3:50 on November 26, during the operation of the automated production line of this automotive parts company, due to a blockage of the second sequence stamping machine, the team leader Zhou contacted the maintenance worker Jiang to repair the equipment mold. Due to the fault not being resolved, Jiang conducted multiple maintenance and debugging operations. At around 4:45, the team leader Zhou reset the total emergency stop switch button of the production line and informed the operator Zhou not to close the 2nd sequence production line door or reset the equipment. Due to the loud noise at the scene, Zhou did not hear clearly and mistakenly thought that the team leader was asking him to close the equipment door. Therefore, without confirming whether the maintenance personnel had left the equipment area, Zhou closed the production line door and pressed the reset button of the 2nd sequence stamping machine to restore the production line, resulting in the 2nd sequence stamping machine mold being repaired by Jiang being crushed to death by the reset starting stamping machine.
One of the main reasons for this accident was the failure to implement “lockout/tagout“. If the maintenance worker Jiang had first disconnected the power and locked the equipment before entering for maintenance, and hung a maintenance sign at the reset button of the equipment, then operator Zhou would not have been able to start the equipment without unlocking the power supply. Seeing the reset button with the maintenance sign, Zhou would have first confirmed whether the maintenance worker Jiang had left the maintenance area, and thus could have avoided this tragedy.
