Why is the actual isolation point more important than the equipment label?
In many factories, one can observe such a scene: on the control cabinet of the equipment, there is a beautifully printed label that reads “Please cut off the power before maintenance”. Then the maintenance worker presses the local button, hangs the lock, and starts the work – and then the equipment suddenly starts. The accident investigation reveals that that machine, apart from the main power supply, also has a bypass power supply, and the label makes no mention of this.

The label is correct, but the mistake lies in regarding the label as the isolation itself.

The label is information, while the isolation point is a physical fact.
These two concepts play completely different roles in the LOTO system. The label addresses “telling you what to do”, and it is essentially a communication tool – informing you of the presence of danger, the need for locking, and who is responsible for it. While the isolation point addresses “preventing energy from passing through”, it is a physical fact – the circuit breaker has tripped, the valve has been closed, the plug has been unplugged, and the energy is firmly blocked outside the working area.

One transmits information, while the other truly cuts off the energy. Information can become outdated, incomplete, or misinterpreted, but the physical isolation does not lie. That’s why all the LOTO standards worldwide consider “energy isolation” as the core action, and the label as an auxiliary means – when the label conflicts with the actual isolation state, it is always based on on-site verification.

Equipment is “living”, while the label is “dead”.
The label also has an inherent defect: it is a snapshot at a certain point in time. After equipment is tagged, renovated, equipped with a frequency converter, additional bypass circuits are added, new pipelines are connected – each time there is a change, the physical reality changes, but the label may not follow. Industry statistics show that for equipment in service for over ten years, the proportion of those whose original electrical drawings do not match the actual situation is astonishing. A label printed three years ago can’t guarantee that it still describes all the energy sources of this equipment today.

More covertly, there is the issue of multiple energy sources. A seemingly ordinary mixer may have five energy sources: electric drive, hydraulic storage, pneumatic assistance, gravitational potential energy, and residual heat. The label usually only indicates the most prominent one. The remaining energy needs to be confirmed by people along the pipelines and cables, one place at a time. This confirmation object is the actual isolation point, not the paper on the cabinet door.

“Locking in the right place” is much more difficult than “placing the label correctly”
In LOTO accidents, a considerable portion is not “not locking”, but “locking in the wrong place”. The lock is placed on the local switch, but the upstream distribution cabinet is forgotten; the lock is placed on the main valve, but the bypass pipeline is ignored; the lock is placed on the equipment-side terminal block, but the maintenance power socket is overlooked. The common point of these accidents is: the label process is not omitted at all, the tag photos are taken neatly, but the physical energy path has not been cut off at all.

The true isolation point has an iron standard: it is the necessary path for energy, locking it cuts off all downstream paths simultaneously. To determine whether a point is a real isolation point, one cannot look at how it is drawn on the drawing or how it is written on the label, but must go to the site to verify – walk along each energy path, use instruments to measure at the operation point, and confirm zero energy state. This is the industry’s repeated emphasis on “verification after locking”: the moment the lock is hung is not the end, the moment the reading is zero is the actual end.

The value of the label is precisely based on the reliability of the isolation point.
This is not to say that the label is not important. The label performs four functions: identification, warning, traceability, and communication, and is an indispensable part of the LOTO system. But it is necessary to recognize their hierarchical relationship: the label is the map, and the isolation point is the road.
