The locking procedure must be audited by the department head to ensure that it is being carried out. The industrial Safety Officer should also spot check the procedure.
Review the content
Are employees notified when locking?
Are all power sources turned off, neutralized and locked?
Are locking tools available and in use?
Has the employee verified that the energy has been eliminated?
When the machine is repaired and ready to start
Are employees away from machines?
Are all tools cleared?
Is the protective device back in operation?
Is it unlocked by a locked employee?
Were other employees notified of the release of the lock prior to resuming operation?
Are all machines and equipment and their locking procedures and methods understood by qualified employees?
Audit frequency
Internal audits by department heads should be conducted at least once every 2 months.
The Safety Officer shall also review this procedure at least 4 times a year.
exceptions
If the shutdown of gas, water, tubing, etc., will affect the normal operation of the plant, this procedure may be suspended with the written approval of the department manager and the appropriate and effective protective equipment provided by the employees.
When it is necessary to find out the cause of the intermittent failure of the machine while it is in operation, this procedure may not be implemented temporarily with the written approval of the department manager and with sufficient safety precautions.
